The CIS website has information on every type of form, addresses for CIS offices, information on fee changes, and a place to order forms online or through the mail.

If you have an emergency, or can’t get the help you need from these resources, Ginny might be able to help. However, Ginny needs your written permission to help with most CIS matters. You can the Authorization Form and fax it or mail it to the Brooksville office.
